LVHN - Coordinated Health is pleased to offer complimentary PIAA sports physicals for our athletes. Fall Sports physicals will be held at the High School nurse's office on Monday June 9th beginning at 4:30pm to 7:00pm

Football -4:30-5:15 Grades 7-12

Boys and Girls Soccer -5:15-5:45 Grades 7-12

Girls Volleyball - 5:45-6:10 Grades 7-12

Cross Country- 6:10-6:30 Grades 7-12

Girls Tennis -6:30-7:00 Grades 9-12

Golf -6:30-7:00 Grades 9-12

The Physical Form is attached. (please use this form)

Sections 1-5 of the physical form must be completed and turned in at the time of the physical. Section 6 will be completed by the physician. All students will need to have a completed physical on file to be able to start Fall sports practices.

If you wear glasses or contacts please bring them for the vision screening portion of the exam.
