Please print and complete the Application for Work Permit below. The student should bring the completed form with a copy of the student's birth certificate, passport, baptismal certificate, or other proof of birth date to the Jim Thorpe Area High School main office. Office personnel will process the application and issue the Work Permit. Students must be 14 years of age to apply for the permit.

Acceptable Documents include original birth certificate, hospital record, baptismal certificate, passport, or Permanent Resident registration, driver’s license/permit, or state ID card with date of birth listed. 

When received, the student should sign the work permit and show it to the employer, who will make a copy for their records. The student retains the original work permit.
